Pharmacology

Carbonic anhydrase inhibitor

Contraindications

General
  • Contraindicated in patients with known sensitivity to this type of drug.
  • Contraindicated in patients with liver or kidney failure, hypoadrenocorticism or those with electrolyte imbalance.
  • Contraindicated in patients with severe pulmonary disease.
  • Long term use is contraindicated in chronic angle closure glaucoma.
  • Use with caution in patients with pre existing blood disorders and diabetics.

Adverse Effects

General
  • Vomiting, diarrhea and loss of appetite are possible side effects of this medication. Give with food to minimize this effect.
  • Sedation, excitement, depression, bone marrow suppression, crystalluria, dysuria, polyuria, electrolyte imbalance, liver insufficiency, rash and hypersensitivity reactions are also possible.

General Interactions
  • May cause false positive urine protein test results
  • May decrease iodine uptake by the thyroid gland in hyperthyroid or euthyroid patients

Corticosteroid
  • Corticosteroids may exacerbate K+ secretion – especially significant in patients on digitalis.
Diuretic
  • Diuretics may exacerbate K+ secretion – especially significant in patients on digitalis.

Drug Interactions
Amphotericin B
  • Amphotericin B may exacerbate K+ secretion – especially significant in patients on digitalis.
Corticotropin (ACTH)
  • Corticotropin may exacerbate K+ secretion – especially significant in patients on digitalis.
Insulin
  • Rarely can interfere with hypoglycemic effects of insulin
Methenamine
  • May negate the effects of methenamine compounds in the urine.
Methotrexate
  • Can cause alkaline urine and affect excretion rate of methotrexate.
PHENobarbital
  • Can cause alkaline urine and affect excretion rate of PHENobarbital.
Phenytoin sodium
  • Phenytoin concurrently can cause osteomalacia
Primidone
  • Primidone absorption can be inhibited.
  • Primidone concurrently can cause osteomalacia
Procainamide
  • Can cause alkaline urine and affect excretion rate of procainamide.
QuiNIDine
  • Can cause alkaline urine and affect excretion rate of QuiNIDine.
